We want to hear from you. Compliments and concerns can be shared in person, by phone, or in writing.
Download our fillable Compliments and Concerns form here. You can return the completed form to the Manager at the site that you visit. Alternately, please email the form to our Director of Clinical Services (dcallan@fivecounties.on.ca).
If You Have a Compliment:
- If we are doing well, let us know so we can build on that. We are constantly working to provide the best possible service to our Clients and Families.
If You Have a Concern:
- Please speak with your clinician to find a solution.
- If this does not work, or you are uncomfortable talking with them, ask to speak with their Manager.
- If you feel that you need more support, you may request a meeting with our Director of Clinical Services.
- If the situation has still not been resolved successfully, a meeting with our Chief Executive Officer can be called to assist with problem-solving.
