We are interested in your family’s experience at Five Counties Children’s Centre.  So we invite you to participate in our 2022 Customer Satisfaction Survey. In particular, we would like to know about your perception of the service you and your family has been receiving from our Centre.

By completing this brief questionnaire you can help us to improve our services. All responses are kept confidential and will not affect the services received at the Centre.

Compliments and Concerns

We want to hear from you. Compliments and concerns can be shared in person, by phone, or in writing. You can download our fillable Compliments and Concerns form here.

If we are doing well, let us know so we can build on that. We are constantly working to provide the best possible service to our Clients and Families.

If you have a concern:

  • Please speak with your clinician to find a solution. If this does not work, or you are uncomfortable talking with them
  • Ask to speak with their Manager. If you feel that you need more support;
  • You may request a meeting with our Director of Clinical Services. If the situation has still not been resolved successfully;
  • A meeting with our Chief Executive Officer can be called to assist with problem-solving.
